Emergency and Health History Form
The Emergency and Health History Form must be filled out by parents or guardians for each student every year. This information is collected to assist JHCS in identifying appropriate care should an emergency occur.
Forms for the 23-24 school year must be completed and returned to Donna Tully ([email protected]) by August 15, 2023.
Activity Hours Form
If a student is fulfilling the JHCS Activities Requirement through an independent activity, this form needs to be turned in to the Director of Academic Systems and Registrar, Donna Tully. Please see the Student-Family Handbook for details.
Community Service Form
JHCS requires that all juniors and seniors complete at least 30 hours of community service independently.
